    Nocters can make our clinics much tougher . They take all the easy urti , rashes, pill checks. My clinic is then filled with patients on 3/4 th course of abx for UTI (usually given by noctor , no Ix instituted) complex mental health , recent hospital discharges with no diagnosis etc etc .all in 10 min intervals .Not to mention the sometimes inappropriate advise an inexperienced noctor can give for eg advising young patient with short hx of typical IBS symptoms they must request referral to get an upper and lower gi endoscopy!

    And why do we need even more patient feedback ? Patients already can complain through the practice directly , NHS choices , ombudsman , NHS england , GMC , iwantgreatcare etc etc and they make full use of all these methods causing enormous stress !!

    If they are unhappy with the referral , then get the senior partner to contact the patient , reassess them and deploy a different management plan if they wish .Otherwise toodles!

    I blame the culture of enforced reflection induced by Revalidation and Appraisal , which is now blatantly a formative assessment . Doctors are naturally reflective and introspective . Now we are forced to constantly self immolate due to the pressure induced by appraisal .This constant searching for negative aspects of practice and relentless self questioning cannot be good for ones mental health , especially considering the baseline pressures of the job .

    To Referral Rejector :NEVER, EVER REJECT MY APPROPRIATE REFERRAL AGAIN OR YOU WILL SUFFER CONSEQUENCES THE LIKES OF WHICH FEW THROUGHOUT HISTORY HAVE EVER SUFFERED BEFORE. I AM NO LONGER A HAPLESS GP THAT WILL STAND FOR YOUR WORDS OF REJECTION & REFUSAL. BE CAUTIOUS!

    Does no one see why there is a recruitment problem . The job has been made unbearable with increasing ltigation and complaints culture , huge indemnity fees ,not to mention the bureaucratic garbage of Revalidation that is triggering early retirements !

    Why would any overseas doctor want to work in the UK ? Facing torturous Revalidation and all the other litigation complaints culture . Doctors wanting to work here must be deluded .

    The sad truth is that a GP is not really viewed as a valuable asset anymore .Noctors and Physician Asociates can be used to mop up a large amount of the daily dross that GPs face at cheap cost . Yes , they may make more clinical errors and there will be more significant events but the monetary gains will keep the politicians and suits rubbing there hands with glee . GP will be left to burn out dealing with the complex multimorbid high risk community patients bounced by the noctors!

    Is the GMC an institutionally racist organisation ? I think it needs to be explored urgently by an independent committee. How else can you explain the astounding excess of black and ethnic minority doctors being called up for GMC investigation . Reading through case files at MPTS shows how stark the reality is for BME doctors .

    International doctors from the south asian subcontinent should be PROACTIVELY informed and advised about this before being recruited to the UK , so that they could make an INFORMED DECISION as to whether they really want to work in this climate !

    Also , sad as it is for me to acknowledge this , I fear there is a pervasive racially determined lean to complaints directed against south asian doctors in the NHS in general . I think this may be a case of unconscious bias in our colleagues as well but it is a significant issue that should be explored .

    Being an asian or black male doctor here in the UK is a serious risk factor for complaints -sad truth!

    Seems like being an asian male GP here is a serious risk factor for complaints !

We are under pressure to always state ' what could I have done better 'even if nothing could have been done differently ! Otherwise we will not pass ARCP panels .The same is now expected in revalidation portfolios and appraisals.

    Hence these self immolatory reflections have incriminatory phrases and elements which can be used against us in court , especially if viewed by lay people /lawyers.It is rich fodder to incriminate !They could be construed as statements of culpability by ruthless lawyers/GMC

    I blame Revalidation (lose your job if you dont comply ), a vicious and unrelenting GMC(damned if you ,damned if you don't) , CQC , stifling complaints culture , exorbitant indemnity as the factors leading to this , among others .
