Of course since 2004 the 'list' has been a practice list - not individual - now if only we could turn back time and re-establish the old lists that would automatically do the trick - oh hang on silly me the lists never got changed they are all still in pre-2004 mode so every patient does have a named GP - they may not like them but they are there.
Now if only we can get a handle on this co-ordinating care malarky that seems such a good idea its a wonder we dont do it already ?

Patients being trained for past 5 years ring your GP anytime you will be transferred to appropriate help, OOH services set up knowing local conditions and facilities generally working well. Now lets give them a nice easy number to remember - but for what?
