    Reduced quality service
    You cannot have your cake an eat it
    i.e. expect the service standards to remain high whilst funding and staff levels fall..

    oh well let the public scapegoat gps for government failings
    I blame austerity
    (stealth austerity in this country)

    Looks like CQC would like more fuel with which to burn practices

    NHS CHOICES website complaints has been around for a long time. Good place for malicious feedback

    Return the services to the remit of the hospital departments..

    Infectious diseases unit...sexual health

    Gynaecology to get Family planning.. complex cases.. procedural cases

    Falls clinic... ENT or Elderly care..

    The lions share of funding goes to secondary care ...
    So the work will get sent there as required..

    Problem is..it will now take much longer

    ?? Unless you have private cover

    ?? Is this the underlying aim

    Looks like nothings free
    (Excluding taxes)
    No wonder America didn't want Obama care..
    but that was on their rationale of keeping taxes low??

    How long till the next general election??

    Will there be anything left to salvage by then??

    The national debt and serving the interest on it

    - Loss of continuity
    - Increasing patient
    frustration
    - Exacerbation of condition
    - End result A+E..
    - Discharge ASAP..
    - Chronic issues
    sent back to GP
    Repeat cycle..

    no new funding = no new improvements
    but by the time anyone notices
    they should have creamed away a healthy profit

    You need to analyse why no one wants to work in these practices
    1. Deprived areas and demanding patients
    2. practices rum with bare minimum staff
    one doc one nurse
    3. unlimited liability for the doctor
    4. excessive workload with admin tesrt results letters
    never ending treadmill
    5. limitless home visit demand

    How to correct
    funding needed so pure sessional work only
    as per locum
    hand back responsibility for home visits to alternative service

    Fund pure admin role for test results and letters

    Stipulate no single handed cover
    minimum 2 docs on site at all times

    only then will things work

    There i saved the NHS thousands in management consultant fees..

    so that whilst the system is starved of funding and things inevitably start to collapse
    the blame will sit firmly with the mnamed responsible gp
    and never go back up the chain to government
    ...

    how about ending this "named responsible gp"
    and the limitless liability this carries
    and change it to
    "named responsible practice"

    it's sad but the numbers in gen prac go up when secondary care is considered worse..

    i.e. turned away from hospital rather than attracted to general practice

    The lesser of two evils ??
    where are you less likely to be scapegoated for organisational pressures

    A consultant surgeon once mentioned its not paradise and easy street on getting to consultant
    but its a long slog in a lot of specialities
    with lots of dues to be paid


    The partnership with all its benefits was the main attraction in the past
    but not now
    with unlimited liability and long hours
    and never ending bureaucracy and external costs
    Salaried - all the work and none of the benefits


    Advise to kids
    Think twice about Medicine altogether
    and if you do enter ..

    SpecialiSe in a speciality
    with better private work capacity
    better career progression / earning potential / Work life balance / Opportunites abroad

    or become a medical management consultant for the NHS.. heard they get paid tonnes

    Easy to say when the prescriber carries medico-legal risk for complications and side effects
    keep it safe
