• What's the NHS Alliance's priority for 2008?

To give GPs and other clinicians a stronger voice in improving the NHS.

• Is practice-based commissioning going to sink or swim?

Swim. We have to make sure it does.

• Why did you decide to advise health minister Lord Darzi on PBC?

Because he asked me, and we can make a huge difference if we get it right. If not, we will be vulnerable to corporate takeover.

• If you were made health secretary for a day, what would you do?

Declare a truce with general practice and create national and local advisory teams of GPs to develop flexible, locally owned plans for better primary care.

• What was your dream job as a child?

To be a politician. Then I realised you need a proper job first and that medicine can make more difference than politics.

• Are GPs paid too much?

Not if they take on additional leadership roles and extend services.

• Should GPs offer extended opening hours?

Yes. This needs to be a local decision balancing speed of seeing someone with ability to access continuing care from a doctor of their choice.

• Who has been your favourite health secretary?

Present secretary of state excepted, it would be Patricia Hewitt. Warm, hands-on and very poorly judged by a deranged media hooked on macho ministerial control.

• What do you do to get away from it all?

I put on my wellies and absorb the plants and animals outside my Devon home.

• Where did you last go on holiday?

Fes in Morocco. More travel than holiday but fascinating and edgy.

• What car do you drive?

A B-reg convertible BMW bought in 1986. It's questionable which of us will last longer.

• What's your favourite film of all time?

Casablanca. A film made in a rush and on a budget, but unmentionably romantic.
