Hooray, I aargree to every word except the belief that QOF was in good faith. It was nothing but to control Gp, they new the price of everything but value of none.Humans are not machines and nothing in treatment can be standardised. The QOF has made our profession to do things but not listen to the most important person-the patient.
Excellent totally agree with prof Gerada. I meandered through various specialities before having my GP training.It has done me a lot of good. I find new GPs excellent in theory but with little practical knowledge of various specialities.

E-referrals, I thought we do them by C&B, It may more appropriate if the government would help GP note to be moved from practice to practice electronically rather than printing all leeters and documents including consultations everytime the patient changes doctors. Even when the patient dies the notes have to be printed and stored by the health department. Can they not do this electronically when most GPs in comparison to hospitals are more electronically tuned. Letters we send will be printed for clinics any way

I have always told my patients that some idiotic rulings are not mine and some very highly educated political yes men. I have to do it and I will has been told but it makes no medical sense to me.I leave it to the patient to say yes or no. If he says no I freely exception report these at patients request. I am sure with this new idiocy Ill have to be exception report more-to the detriment of patients care? or who is bothered with the patient as far as all boxes are ticked. This man personifies everything that is wrong with the NHS. It is the process that is important rather than the outcome. Who cares what the patient wants,send him a choice of 4 hospitals they say 12 week indicative time and when the patient rings within a few hours ir is 18 weeks.The old system of writing to consultants and they deciding depending on the urgency was any day better. Please Lord Howe scrap this stupid waste of time and lets us work to the patients benefit rather than your stupid ideas.

i for one am quite happy. Are we all not professionals and can we not do the job properly. I am sure most us were doing it properly before this tick box culture came in. The amount of communication between HCPs has got worse since this came in. As for patients if it is ticked it is right. Patients are humans and not machines if this is wrong give taht culture should end and the lead professional, Gp or palliative care nurse should decide on an idividual basis and not follw like an alogrithm monkey
