Dr Stephen Fox plans to aspire to maximum points after setting up an annual recall system for all patients on disease registers covered by the framework. Dr Fox, a singlehander in Leigh, Lancashire, said: 'If you get them all done in their birth month it really ensures you will extract maximum amount.

'It's minimal effort on our part, because the secretary just picks up the birthdays, the patient gets a letter, and comes to get their blood pressure taken. Then you can ask about smoking and so on.'

Dr Fox also advised GPs to check changes to Read codes. 'If you have listed a patient for anxiety and depression but quality points are for a history of depressive illness, you must look to see if it is twinned with a depression code. What was a 35 per cent score could become 90 per cent.'
