I work in a care home and we come across this problem often. Residents discharged from hospital care appear to have a spectacular dosage change - sometimes stopped - in essential and regular medication.
<p>
We always check this out with their GPs or OOH - sometimes residents being returned home late at night - as from the moment of return it is their life in our hands.
<p>
Often a list of medication is all we get - no details of problem uncovered etc in a discharge letter. margaret fairlie
