Happy medium: why nirvana is found in mid-sized practices

Congratulations, you are a winner. I knew there must be one somewhere. In the days before CQC assault on practices, I was QOF assessor for CCG and out of every 50 practices visited saw one or two like you describe: commuter region, lots of working age adults, low morbidity, low dementia and MH and not many housebound or care homes. My practice is same size in retirement town, we do up to 30 Home visits a day, quarter of patients aged over 65 (we are losing)
