Should HRT be tailed off and how?

Patients often ask whether they should ‘tail off' their HRT. Is there any rationale for this? And if so, how long should they take? If hot flushes recur after stopping HRT, does this mean the menopause is continuing or is this just a discontinuation phenomenon?
