What are a GP's obligations if they receive a gift from a patient?

During one of our regular consultations I mentioned to an elderly patient that I was planning to have a conservatory built. I have just received a card from her with a note saying: "I've enclosed a cheque as a little thank you to put towards your new conservatory". The cheque is made out to me for several thousand pounds. I'm sure I've done nothing to encourage the gift and the patient has full mental capacity. Would it be appropriate to accept the money?
