Continuity of care



Practical Commissioning’s jargon buster defines the term ‘continuity of care’

This is a term used by GPs to refer to how they build up relationships with their patients over time and become well-informed about their individual patients’ needs.

There has been much concern by some GPs about continuity of care being compromised as the traditional model of general practice changes with patients often now being seen by different GPs within the same practice and as a result of practices now not seeing patients out of hours.
