Practical Commissioning's Jargon Buster explains the meaning of the term 'Horizontal integrated care'.
Horizontal integrated care refers to models of joint working where the NHS works with outside organisations for example local authorities.
Its opposite is vertical integrated care which refers to joined up care within the NHS eg primary and secondary care.
