CSA practice case: Down’s syndrome patient with heart failure

This case refers to the sister of your 44-year-old patient who wants to discuss her brother.

He has Down’s syndrome and suffers from diabetes and heart failure, secondary to an incompetent mitral valve. He has had three admissions to hospital in the last year as a result of worsening breathlessness secondary to heart failure. His sister does not want him to go back into hospital if his breathing gets bad again.
