The government and most of the NHS treat the service like a religion resulting in lots of 'thou shall not' with the result that it sees itself as precious. Instead of abolishing practice boundaries the NHS should treat GPs equally with consultants and allow private practice. The NHS, like private health insurers, needs to have well defined boundaries as to what is covered and conditions that are not eligible.

The circle that has never been squared by the NHS is how to enhance the difference between GP tryin to get to know the patients and Hospital practice trying to know more about rare diagnoses. The cost-effectiveness of seeing the same GP over successive consultations is under-rated in anything other than small partnerships of one or two principals, even if they are using premises shared by other firms. The NHS has the potential to do this better than any other system of health care, especially when dealing with psychological reaction to life events.
