How PCTs are performing on Health Checks

1.      NHS Bury – ‘Failed' on targets for NHS Health Checks RED

2.      Redbridge – ‘not expected' to meet 2011/12 target in Q4 due to ‘late start' on programme RED

3.      Waltham Forest – will not meet 2011/12 target in Q4, ‘more GP engagement required' to improve RED

4.      Havering – improving although was behind in Q3, now with 11.2% offers compared to a plan of 12.2% AMBER

5.      NHS Buckinghamshire and Oxfordshire Cluster – above target on offering checks in Q2, but take-up is 1.8%, compared with target of 2.7% AMBER

6.      NHS North Derbyshire CCG – percentage of eligible people invited is 8.4% against a plan of 12.6% in Q3 AMBER

7.      Newark and Sherwood CCG – behind on target number of eligible patients offered checks in Q3, but take-up is good AMBER

8.      NHS Trafford – improving performance in Q2 but still short of target. 6.5% of the target population have been offered a health check, compared with target of 7.6%. Have approved the recruitment of an additional healthcare assistant for a fixed term of three months to work across primary care ‘to improve volume of checks' AMBER

9.      NHS Cambridgeshire and NHS Peterborough – has been significant improvement after ‘remedial action plan' targeting under-performing practices, but behind in Q3 on number of checks completed AMBER

10.  West Leicestershire CCG – further work is required and co-chair admits achieving the Health Checks target ‘is likely to be one of the biggest challenges to the CCG' AMBER

11.  Kingston Commissioning Committee – ‘significant risk' over the performance of this target in Q2, with 3.1% receiving a health check against a quarter target of 3.8% due to inadequate funding arrangements and the ‘performance of providers, particularly community pharmacy'. AMBER
12.  Barking and Dagenham – achieved targets in Q3 and now on track for Q4 GREEN
