I'd certainly agree that small groups would improve the way that the organisation communicated and managed. Unfortunately, cost effective management relies on organisations being larger. It is the same problem with the move to small, local services rather than big hospitals - no matter what you have read about hospital costs they are far more cost effective than if the same was provided in smaller units. So, sorry the money calls the tune.

I agree with PCT Finance Manager, but I would go on to say that in order to achieve the best outcomes and deliver care in the most efficient way acute services are coming togetherin larger centres. Small local groups will, in most cases, not be in favour of losing services to the large centre and support their local trust no matter how much of a basket case it is. Therefore larger CCG's are required to ensure decisions are made free from political nonsense and in the patients best interests.
