General Practitioner of the Year (sponsored by Exeltis)

Dr Ishi Bains, GP in Tower Hamlets, women’s health hub co-clinical lead and regional primary care lead for gynaecology
As the co-clinical lead for the Tower Hamlets Women’s Health Hub, Dr Ishi Bains has transformed how women’s health services are delivered in the region. Her work has addressed challenges such as excessive waiting times, fragmented care, and inequalities in service provision.
Through the development of an integrated and accessible hub, Dr Bains has improved patient access and continuity of care for women in Tower Hamlets, ensuring that they receive timely and holistic treatment.
Dr Bains has also worked to break down barriers between primary and secondary care to create more efficient and patient-centred services, creating EMIS templates for management pathways based on NICE and Royal College guidelines.
The templates are available to GPs in north east London and have reduced referral rates, improved referral quality, and standardised care; while also serving as an educational tool for GPs and reducing documentation time.
Speaking at the awards ceremony, Dr Bains said: ‘We need to remember that we are one NHS working together. We need to break those barriers between primary, secondary care, community service.
‘Let’s stop being such a fragmented service and work together as one team. I think that collaboration is why we have achieved so much in our women’s health hub.’
She added: ‘I am honoured to have won this award, but I feel like I am representing all GPs who do so much. I feel like I’ve just been the lucky one to be nominated, but I don’t feel like I’m actually the one who has done the most.’
General Practice Team of the Year (sponsored by General Practice Solutions)

Modality East Surrey Medical Practice Team – one Practice, One Purpose, Excellence Through Integration
Merged from three independent surgeries, Modality East Surrey was formed with the aim to deliver equitable access for all patients.
The team now serves over 38,000 patients as a single practice, leading the way through innovation and a strong patient-centred ethos, streamlining care and providing consistent high-quality services.
Modality East Surrey has worked to redesign an ‘inclusive’ front door for patients – both digital and non-digital. This includes empathetic reception support, flexible triage, proactive management of long-term conditions and integrated teamwork to ensure that patients are seen by the right clinician, at the right time, in the right way.
The team’s innovation has increased its capacity and access. Over 48,000 medical requests have been handled in nine months, with 92% being dealt through automation. More than 1,300 appointments remain unused each month, which are now used for proactive frequent attenders review.
GP partner at Modality East Surrey Dr Thiruselvi Bangalore said that winning this award made ‘all the hard work put in behind the scenes worthwhile’.
‘I am so proud of our team. The hard work, the long days and all the difficult bits and bobs every day – an achievement like this makes it feel like we have done a good job.’
